On Friday, September 12, CHI Memorial will be hosting its Mobile Health Program regarding mammogram screenings at Tate Funeral Home in Jasper. It will be on-site from 9 a.m. to 3 p.m., and all patients are required to bring their driver’s license or state I.D., insurance card, completed registration form, and full name of primary doctor.
